Are you passionate about helping patients transition smoothly between levels of care? We're seeking a Part-Time Transitional Care Manager to join our dedicated team!



  • Hours: 20 hours/week, ideally Monday-Thursday, 9am-2:30pm (some flexibility available)


  • Location: Skilled Nursing Facilities located in Dover, New Hampshire


About the Role



As a Transitional Care Manager, you'll play a critical role in coordinating care for patients moving from acute to post-acute settings. You'll collaborate with patients, their families, and healthcare teams to ensure smooth transitions, reduce hospital readmissions, and optimize care plans.



Your Responsibilities Include:


  • Partnering with acute care teams to determine the best post-acute care plans for patients.

  • Building strong relationships with skilled nursing facilities (SNF) and their staff to promote efficient, quality care.

  • Using clinical decision support tools and standard operating procedures to guide care plans and ensure timely discharges.

  • Facilitating communication between care providers and community support services to minimize readmissions.

Qualifications

What We're Looking For




  • Required:


    • Licensed Physical Therapist (PT), Physical Therapist Assistant (PTA), or Occupational Therapist (OT)

    • Bachelor's degree from an accredited program

    • 2+ years of experience in an acute hospital or post-acute care setting

    • Familiarity with electronic medical records (EMRs), clinical systems, and a broad understanding of community hospitals and post-acute services

    • Strong organizational and critical thinking skills with the ability to multitask in a dynamic healthcare environment



  • Preferred:


    • Experience in case management, utilization review, and discharge planning

    • Knowledge of Medicare regulations, especially regarding SNF care

    • Proficiency with Microsoft Excel, PowerPoint, and Word
