Location: Skilled Nursing Facilities located in Dover, New Hampshire
About the Role
As a Transitional Care Manager, you'll play a critical role in coordinating care for patients moving from acute to post-acute settings. You'll collaborate with patients, their families, and healthcare teams to ensure smooth transitions, reduce hospital readmissions, and optimize care plans.
Your Responsibilities Include:
Partnering with acute care teams to determine the best post-acute care plans for patients.
Building strong relationships with skilled nursing facilities (SNF) and their staff to promote efficient, quality care.
Using clinical decision support tools and standard operating procedures to guide care plans and ensure timely discharges.
Facilitating communication between care providers and community support services to minimize readmissions.

2+ years of experience in an acute hospital or post-acute care setting
Familiarity with electronic medical records (EMRs), clinical systems, and a broad understanding of community hospitals and post-acute services
Strong organizational and critical thinking skills with the ability to multitask in a dynamic healthcare environment
Preferred:
Experience in case management, utilization review, and discharge planning
Knowledge of Medicare regulations, especially regarding SNF care
Proficiency with Microsoft Excel, PowerPoint, and Word
