We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.ThePatient Service Representativeis responsible for greeting guests to the health center, completing the patient intake process, scheduling patient appointments, and monitoring waiting room activity.
