Schedule/Location:



Our RN Care Manager Lead will work in a hybrid capacity with supporting our nurse care managers in North Shore Physician Group Primary Care offices. Additional time may be spent in other settings including occasional travel to our corporate headquarters in Assembly Row, Somerville.


Due to the hybrid nature of the role, we ensure that our employees receive required technology and training to be proficient and independently productive in all job responsibilities regardless of work location. Employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked.


The Opportunity:



The Team Lead role is accountable for the delivery of consistent, high quality care management services, promoting the professional development and satisfaction of Care Managers, providing operational efficiencies and insuring compliance with PCP practice, North Shore Physicians Group (NSPG), North Shore Medical Center (NSMC), and Mass General Brigham regulatory policies and standards of practice. This position has first line responsibility for the staff and daily operations including patient focused care management practice, personnel management and development, fiscal responsibility, and partnerships/liaison with numerous clinical and non-clinical services/departments.


Primary Responsibilities to include but not limited to:


  • Provides an environment for effective, patient focused care management and services throughout the inpatient and outpatient continuum for participating patients.

  • Conducts regular audit and observation of Care Managers' work to ensure compliance with CMS Care Management Program Standards of Practice, policy and procedures and regulatory requirements. Provides timely feedback to staff.

  • Coaches and mentors individual Care Managers to promote professional development and to assess opportunities to enhance the Care Manager's role and critical thinking.

  • Monitors and evaluates opportunities for improving Care Manager assignments.

  • Provides daily, direct Care Management support to the Care Managers for complex patients in conjunction with Care Management Leadership Team.

  • Develops and implements initiatives to promote staff development including opportunities for individual and group training and development, promote written exemplars, narratives, case presentations, Partners in Excellence Award, and Case Management Certification.

  • Interviews, hires, evaluates and supervises RN Care Managers (approximately 20.0 FTEs).

  • Responsible for daily, monthly, annual management of the Care Management Team salary component of CMS Care Management Program budget.






Qualifications

Required:


  • Bachelor's degree required; master's degree preferred.

  • Minimum 2-4 years acute care experience required.

  • Minimum 2-4 years progressive management experience required.

  • Other experience may include post-acute care settings or care coordination/case management experience.

  • Demonstrated case management understanding / expertise.
