Job Description:

Location: Marysville, WA

Salary: $38.00 USD Hourly - $40.00 USD Hourly

Description: We are currently seeking a Case Manager RN for a Physical Therapy Clinic!



Hours: 7:30-4 pm: Monday - Friday



Location: The location of work will be both remote and on site days in Marysville, WA 98271



Work type: Contract (possible to hire)



This job will have the following responsibilities:

• 
  


• Hospital Transition calls for Medicare Advantage patients


• Care Coordination for complex, high-risk patients


• Motivational interviewing with patients and families


• Palliative Care conversations when appropriate


• Participation in Interdisciplinary Team meetings
  
  

Qualifications & Requirements:

• 
  


• Ambulatory Care


• Outpatient Care Coordination


• Difficult conversations


• Care Management


• Hospital Transition work

CCM or Palliative Care Certification would be a major plus!
