Overview
Care Coordination, Case Management, System-Level Intervention & Skills Training
Provides community and home-based face-to-face (on-site and in the field, 24/7) and telephonic outreach, system-level intervention, disease and/or care management, care coordination, education, support and care navigation to members participating in the OHSU Health Plan program as well CCO and commercial plan members statewide.
Attends medical appointmentswith patients and caregivers and serves as a bridge supporting implementation of care plans on a daily basis.
Engages providers and the care team in integrated strategic system-level interventions.
Behavioral Intervention Design and Implementation
Conducts assessments, develops plans of care and interventions to address social determinants and barriers to care, and sets goals for plans of care and implements behavioral interventions within their scope of practice.
Works collaboratively with the interdisciplinary team in identifying opportunities for improvement and participating in the development of new approaches to enhance effectiveness in work performed.
Agency Representation & Community Resource Management/Collaboration
Collaborates with Medicaid Plan member’s medical, community-based providers (that may include the primary care provider (PCP), specialist provider, behavioral specialists, mental health providers, school nurses, or others as may be indicated) regarding the member’s treatment needs and plan of care, identifying creative strategies and developing and implementing integrated system-level interventions to address psychosocial issues and healthcare inequities.
Responsibilities
NICH Interventionist 1 Qualifications:
1 year as an Interventionist 1 at OHSU
OR Bachelor’s Degree and 3 years relevant experience
OR Equivalent combination of education and relevant experience
Valid Driver’s license is required
Please note: Consideration will be given for significant life experience that directly enhances the individual’s ability to perform this job. Additionally, retail or hospitality customer service roles will be credited at 50% towards relevant experience.
NICH Interventionist 2 Qualifications:
Associate Degree
OR Equivalent combination of education and relevant experience
Valid Driver’s license is required
