Nemours is seeking a Division Coordinator - Solid Organ Transplant for department of surgery in Wilmington, DE.
Primary Functions:
The Solid Organ Transplant Division Coordinator will support multidisciplinary programs including Liver Transplant, Kidney Transplant and Heart/Cardiac Transplant. The primary focus of this position is coordinating, scheduling across specialties and managing growth of the Solid Organ Transplant Program.
Responsibilities will include but are not limited to:
Maintain up to date patient databases that support each of the programs
Manage EPIC Cadence clinic templates across disciplines
Act as point administrative representative at weekly & bi-weekly Transplant meetings
Recalls are created within the programs
Communication with appropriate authorization groups (i.e., hospital and physician)
Schedule patients within each program under the guidance of clinical coordinator
Communicate with clinical coordinator, connect clinical staff with families as needed
Identify areas for system improvements within each program, proactively problem solve.
For Solid Organ Transplant Program:
Develop, modify and/or maintain database
Create Excel file to track patients presented in weekly clinics
Facilitate weekly Transplant meetings, including securing location and moderating for remote participants
Record patient discussion plan, as well as draft patient summary documents as required by Joint Commission and CMS

Job Requirements:
High School Diploma
Minimum of 3 years of job related experience required.
