Case Management Support Specialist - Rapid Rehousing at LifeWorks Austin

Job Description:

LifeWorks of Austin, TX is looking for a Case Management Support Specialist to join our Rapid Rehousing program! The Case Management Support Specialist works closely with other professionals in coordinating services provided within and outside of LifeWorks. This position builds trusting relationships with individuals that are not being served or are underserved by existing community service delivery systems through engagement and support services. Services include (but are not limited to) supporting clients with the completion of intake process, administrative functions, as well as provision of basic needs, referrals, and crisis intervention. Responsible for providing administrative support to programs by maintaining client files, entering data, and completing required documentation.

This full time, non-exempt position earns a competitive starting rate of $23.00/hr. Benefits are also included, which consist of 24 PTO days per year (accrued); 11 paid holidays per year; a premium-free employee medical plan with $100 per month HSA contribution (plus other affordable employee or family medical, dental, and vision options!); a retirement savings plan with 50% match on the first 6% of contributions, and much more!

RESPONSIBILITIES


  •  Provide client-focused, healing-centered case management support services diverse individuals who may have experienced chronic homelessness, systems involvement, diagnosed mental health issues, substance misuse, a history of trauma, early pregnancy/parenthood, etc.

  •  Maintain timely, complete, and accurate case records and documentation.

  •  Perform heavy data entry into agency/funder databases and ensure timeliness and accuracy of data entry.

  •  Assist clients with obtaining necessary documents to secure employment or housing (e.g., state ID, birth certificate, social security card).

  •  Provide individualized services to clients that are primarily administrative in nature, such as completing paperwork or online applications. 

  •  Regularly and effectively assess and manage incidents and crisis situations with calm, care, and positive regard for clients, using de-escalation and safety planning techniques as appropriate.  

  •  Comply with all requirements outlined by funding sources, licensure, and accrediting bodies, the program, and the agency. These requirements may pertain to data, paperwork, philosophies, processes, and procedures, among other things.

  •  Collaborate with internal and external partners to advocate for client needs, coordinate services, and provide referrals.

  •  Transport clients to agencies and services, as needed.

  •  Prepare for and actively participate in supervision meetings and all other required meetings or trainings.

ADDITIONAL ESSENTIAL DUTIES AND RESPONSIBILITIES RAPID REHOUSING: 


  •  Closely monitor program compliance by completing monthly file reviews.

  •  Manage and contribute to the community resources folder for case manager to access for clients.

  •  Provide temporary coverage and direct client services during staffing transitions or outages on an as-needed basis.

  •  Request rent checks for clients and, on an as-needed basis, deliver them to property managers.

  •  Support the move-in process on an as-needed basis by attending lease signings, assisting with furniture deliveries, and transporting clients' personal effects.

QUALIFICATIONS 


  •  Requires a Bachelor’s Degree in social or behavior sciences or administrative field;

  •  Requires a minimum 6 months experience with youth; or

  •  Equivalent combination of education and experience.

  •  Proficient in HMIS, Microsoft Office, Outlook, and agency database.
