The Director of Patient Services (DPS) is responsible for ensuring that patient care is coordinated and managed appropriately in response to needs of patient / family / caregiver. This role ensures that care and services are delivered, documented and oversees the supervision of clinical personnel. The DPS is responsible for clinical education and quality review and will facilitate the QAPI program including management of on-going clinical staff development initiatives. The role serves as a resource for the development of standards of patient care by implementing evidenced-based practices or best practices.
RESPONSIBILITIES
Conferences with Medical Director regarding questions about an individual's eligibility for services
Reviews and consults with clinical staff regarding patient's medical diagnosis, prognosis, medications, procedures and clinical course
Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care
Assists staffing and scheduling through communications with other managers and support staff, reviews time sheets (time logs) as appropriate. Is responsible for the assignment of adequate and appropriate staff
Facilitates communications and works effectively between hospice staff, patients / families / caregivers, physicians, vendors, and other departments and professionals as appropriate
Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of Interdisciplinary Group involvement
Conducts quarterly record reviews and communicates findings and recommendations to the QAPI Committee and Director of QAPI
Assists the Administrator and other hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs
Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range
Assists in recruiting, hiring / terminating, evaluating and manages members of the team; orients new personnel and arranges for in-service education, as appropriate
Complies with accepted professional standards and principles
Assesses health needs of community and assists in the development of plan(s) / program(s) to meet needs. Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all hospice personnel
Actively participates in the Quality Assurance and Performance Improvement program
Performs other duties and activities as delegated by the Administrator and / or senior management
Accepts direct responsibility for the growth and overall success of the Company
Is available for consultation at all times during operating hours of the agency
Meets productivity expectations for this position; organizes tasks and manages time effectively
Can work independently and alone, without supervision. Asks appropriate questions when unsure of an assignment, takes initiative in areas expected of him/her. Uses good judgment in difficult situations

Driver for quality initiatives based on measures and outcomes, process, and
Manages data collection for key quality indicators, including: incident reports, Comprehensive Clinical Record Review and Admission Audits
Trend analysis of results
Consolidates results
Creates and or facilitates quarterly QAPI Report
Facilitates continuous improvement processes specific to regulatory compliance
Identifies needs/opportunities for improvement
Develops action plans
Oversees implementation of plans
Staff Development
Orients new clinical staff
Drives ongoing clinical education
Keeps up to date on changes related to regulations and COP's
Assists in implementation of any changes
Supports mission, values and goals of Signature Hospice, Home Health & Home Care
QUALIFICATIONS
Registered nurse with current licensure to practice professional nursing in the State preferred, (other clinical certification/licensing considered)
Previous experience in hospice setting and with two (2) years management or supervisory experience preferred. Proven ability to work within an interdisciplinary setting
Demonstrates excellent observation, good nursing judgment and verbal and written communication skills
Understands hospice philosophy and issues of death / dying
Understands principles of pain / symptom management
Must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements
Complies and maintains current CPR certification
Possess basic computer skills and knowledge of office technology - e.g. email and internet services, facsimile machines, telecommunication devices,
Must have a passion for developing and mentoring people
Ability to communicate information and knowledge that is compelling and relevant to the target audience
