BCForward is looking for Care Management Support Coordinator II (Remote)

Pay Rate: $25/hr on W2




**Position Summary:**



The Administrative Care Management Support role is responsible for assisting with various administrative tasks related to care management. This includes performing outreach to members, answering inbound calls, scheduling services, and serving as a point of contact for members, providers, and staff to resolve issues. The role requires documentation of member records in compliance with current state and regulatory guidelines.



**Key Responsibilities:**



- Conduct outreach to members via phone to support care plan next steps, community or health plan resources, scheduling concerns, and ongoing education for both members and providers.


- Provide support to members in connecting them with health plan and community resources to ensure high-quality customer care/service.


- Serve as the front-line support for member and provider inquiries, requests, or concerns, including explaining care plan procedures and protocols.


- Support member onboarding and handle day-to-day administrative duties such as sending welcome letters, correspondence, and educational materials to facilitate a successful member/provider relationship.


- Document and maintain non-clinical member records to ensure compliance with state and regulatory requirements and provide information to providers as needed.


- Apply working knowledge of assigned health plan activities and resources to address Social Determinants of Health (SDOH) needs.


- Comply with all policies and standards and perform other duties as assigned.



**Education/Experience:**



- High School diploma or GED required.


- 1 - 2 years of related experience preferred.
