Job Title: Registered Nurse Care Coordinator/Case Manager (Genetics)

Location: Durham, NC 27701
Work Hours: Business Hours: Monday to Friday, 08:00 - 16:30 ET (Closed on weekends and all Federal Holidays)
Job Description:
Triage all incoming consults within two business days of receipt.
Contact patients with negative results within three business days of receipt.
Maintain a record with no more than one substantiated complaint from patients or staff.
Update the patient tracker with relevant information accurately and with minimal errors.
Ensure consults to other services are input and ready for signature.
Follow all established medication policies and procedures; no sample medications to be provided to patients.
Provide discharge education and follow-up instructions coordinated with the next care setting for all Hematology/Oncology clinical patients.
Qualifications:
Registration: Must be a registered nurse (RN) with a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth
Education: Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the recognized accrediting bodies at the time the program was completed.
Certifications: All staff must be currently certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS), or equivalent.
