Job Description:

Nemours is seeking an RN for our Nurse Navigator-Hematology oncology opening in Wilmington, DE.

Primary Functions:

This position is responsible for coordination of a high level of service and quality of care to individuals requesting outpatient service for the Lisa Dean Moseley Foundation Institute for Cancer and Blood Disorders (LDMFI-CBD) and sub-specialties where patients of the LDMFI-CBD are treated. The Leukemia Nurse Navigator will serve as an advocate for complex patients, their families, and caregivers, to overcome health care system barriers.

The primary functions include:


  • Improve patient's access to care by assuring efficient and effective coordination of patient care between visits including scans, complex scheduling, and sub-specialty interface.
  • Triage newly diagnosed patient and family with complex needs in the outpatient setting and provide procedure related information during the scheduling process.
  • Collaborate with the physicians, members of the healthcare team, and the patient/family to maintain a plan to deliver quality healthcare in the outpatient setting and in the home environment, focusing on educational and resource needs.
  • Provide guidance and education for the highly complex patient and family to ensure the patient receives the necessary care, throughout the continuum of care.
  • Coordination and delegation for prior authorizations and peer to peer for highly complex patients.
  • Collaborate with the LDMFI-CBD specialty nurses for care coordination and cross coverage as needed.
  • Evaluate the patient/family response to care on an ongoing basis.


Essential Functions:

  • Assists with all aspects of navigating the patient and family through the health care process assuring that continuity of care is promoted in all patient care settings.
  • Proactively develops and enhances processes with the goal of improving the clinical experience for referred patients and the referring physician.
  • Participates in multi-disciplinary team discussions in relation to the individualized patient care plan.
  • Bridging gaps in care and supporting care coordination for patients with highly complex care needs.
  • In collaboration with the outpatient nursing team, serves as a liaison between the patient/caregiver and provider when necessary.
  • Provide clinical assessment expertise to scheduling parameters - timing of visit, appropriate provider, etc.
  • Serve as an educator/resource on divisional protocols for complex outpatient appointment scheduling, patient triage, provider expertise, and other details which are pertinent to accurate appointment scheduling.
  • Collaborate with research team for patients enrolled in clinical trials
  • Maintain membership Children's Oncology Group (COG)
  • Complete and maintain CITI training
  • Maintain all necessary licenses, education, etc.
  • Participates in program, hospital wide CQI initiatives.
  • Completes mandatory in-service training yearly which includes but not limited to safety, infection control, HIPPA, corporate compliance, and bioterrorism.
  • Maintain Association of Pediatric Hematology Oncology Nurses (APHON) Chemotherapy/Biotherapy provider status.
  • Other duties as assigned.

Required:


  • BSN required
  • Current, unrestricted DE Registered Nurse License required (subject to change based on patient base).
  • BLS and PALS through American Heart Association required
  • Minimum 5 years of pediatric hematology/oncology nursing experience required.
  • CPON/CPHON certification preferred.
  • ANCC approved professional board certification specific to clinical practice; must be obtained within 1 year of employment:
