Job Description:

TITLE: Nurse Manager
REPORTS TO: VP of Home and Community Based Services

MINIMUM QUALIFICATIONS

1. Is a graduate of an accredited school of professional nursing and is currently licensed as an
RN through the State Board of Nursing.
2. Prefer at least one year's clinical experience, with current Advantage nursing experience
and Harmony experience.
3. Complies with accepted professional standards and principles.
4. Possesses and maintains good physical stamina and mental health. Has presented a pre-employment health clearance.
5. Is a licensed driver with an automobile in good working order and has insurance in
accordance with state and/or Agency requirements.
6. Is flexible and cooperative in fulfilling role obligation.
7. Has excellent observation, good clinical judgment, and good oral and written communication
skills.
8. Is self-directed with the ability to work with little supervision; has good organizational skills;
and is able to work cooperatively with others.

SUMMARY OF JOB RESPONSIBILITIES

1. Provides quality and safe advantage nursing services to clients and families/caregivers.
2. Implements plan of care initiated by the RN, case manager and/or physician.
3. Provides accurate and timely documentation consistent with the plan of care.
4. Assesses and provides client and family/caregiver education and information pertinent to diagnosis, plan of service and plan of care.
5. Participates in coordination of care based on advantage/AAU conditions of participation.
6. Uses equipment and supplies effectively and efficiently.
7. Performs duties within the limits of preparation and experience.
8. Seeks and obtains sufficient information necessary to function effectively in the health care
setting where assigned.
9. Provides professional care by utilizing all elements of nursing process and/or agency
standards of care.
10. Assesses and evaluates the clients' status by:
A. Reviewing and implementing the plan of care
B. Regularly re-evaluating client needs
C. Coordinating and communicating with other professionals or agencies involved with
the client.
D. Recommending plan of care revisions to the agencys Case Manager.
11. Initiates and applies appropriate preventive, therapeutic and rehabilitative procedures and
techniques and maintains communication regarding client care with the physician, and other
disciplines, or agencies, involved in the care.
12. Completes, maintains, and submits accurate and relevant clinical notes regarding the
clients condition and care given.
13. Observes the client for changes in condition, discusses changes with the client or
family/caregiver and reports changes, as necessary, to the attending physician and RN case
manager.
14. Evaluates the clients ability to perform activities of daily living and facilitates the client's
efforts toward self-sufficiency and independence.
15. Assists the client in maintaining an optimal, safe environment.
16. May train and/or supervise aides or LPNs, at the direction of the VP of HCBS.
17. Actively participates in Quarterly QAPI meetings, assisting the team to determine the
appropriate corrections and performance improvements needed for the nursing portion of
our Advantage/home care program.
18. Supervises the client care provided by non-licensed personnel, including HHAs and PCAs most often done during in home supervisory visits.
19. May travel to other offices in the state to assist with visits or submitting documentation to the
AAU/DHS as needed.
20. Nurse manager will supervise and hold accoutnable all nursing staff that falls under the
21. Nurse manager will monitor and process DARs daily to ensure they match the nursing
notes and poc, ensuring that all processed DARs are sent to billing for processing.
22. Nurse manager will help ensure all nursing visits are completed timely to include SOC and
6-month SUP visits. Helping with scheduling of visits when needed.
23. Nurse manager will run report daily to ensure all 5 day follow ups are being done by
assigned nurse.
24. Nurse manager will ensure all notes are checked and correct prior to submitting to
AAU/DHS.

1. Visual/hearing ability sufficient to comprehend written/verbal communication.
2. Ability to perform tasks involving physical activity, which may include heavy lifting and
extensive bending and standing.
