Schedule:
Full-time (Monday-Friday, 8am-5pm)
No weekends, holidays, or on-call

Responsibilities:
Care for high-risk patients in senior living facilities
Conduct assessments
Manage medical and behavioral conditions
Collaborate with care team
Educate and counsel patients
Discuss advanced illness and care planning
Maintain accurate medical records
Qualifications:
Active state license (NP/PA)
Master's in Nursing (NP) or Physician Assistant degree (PA)
National board certification (specific certifications listed)
Experience with complex patient needs
Strong communication and collaboration skills
Proficient in electronic medical records
COVID-19 vaccination required
