Job Description:

Location: San Francisco, CA

Salary: $150,000.00 USD Hourly - $170,000.00 USD Hourly

Description: Our client is currently seeking a NP or PA - San Francisco


*This is a full-time, direct-hire role with a great salary and benefits



This job will have the following responsibilities:

• Conduct initial and annual face-to-face patient visits in home and around the community


• complete patient Health Risk Assessments


• Update and reevaluate the year-to-end diagnoses following care


• Identify diagnoses and develop the proper treatment


• Collaborate with HEDIS/STARS


• Complete EMR Documentation with quality


• Conduct patient referrals


• Other duties as assigned

Qualifications & Requirements:

• Active California State License as an NP or PA


• Computer Literacy/EMR experience


• 1 year of clinical experience


• Valid Drivers License


• BLS and ACLS


• DEA number (within 6 months of hire)


• NPI Number
