Job Description:

Salary: $25.50 - $36.80 Per Hour


 


SUMMARY


The Enriched Residential Care (ERC) is a program under DHS’ HFH and provides housing for clients who have complex medical health, mental health and substance use disorders and require care and supervision in licensed care facilities.  This position will provide administrative and technical support to the ERC Program Managers.


 


ESSENTIAL FUNCTIONS



  • Familiarity with CHAMP environment and review CHAMP referral queue daily to respond to incoming referrals.

  • Review referral for completeness and eligibility criteria.

  • Coordinate the completion of the physician's report with the clinical team, if necessary.

  • Coordinate TB testing, if necessary.

  • Move eligible clients to the assessment queue and notify the referring entity.

  • Contact referring entities to alert them if the referral is incomplete or missing documents. Provide technical assistance to referrers if needed.

  • Log all eligible referrals into Master Tracker.

  • Remove referrals from the CHAMP queue if still incomplete after 30 days.

  • Assign client to appropriate ERC Project in CHAMP

  • Confirm client income, through LRS, such as SSI, SSDI and CAPI.

  • Confirm health insurance (check board)

  • Collaborates with Department of Public Social Services (DPSS) Liaison and County-wide Benefits Entitlement Services Team (CBEST) and Medical Case Workers (MCW) clients’ benefit establishment and income verifications.

  • Generates Changes Income Forms to track client’s Social Security benefit eligibility.

  • Assists ERC facility vendors on Mental Health Resource Locator and Navigator (MHRLN) Bed Tracking System registrations.

  • Update case notes in CHAMP.

  • May participate in MCW and program managers case conference calls.

  • Other duties as assigned.

JOB QUALIFICATIONS



  • Affluent with numbers and strong proficiency with Excel software required, familiarity with other data processing and visualization software is an advantage.

  • with professionals from multiple teams and fields of work.

  • Ability to collect and process data from a variety of sources and produce clear and concise recommendations.

  • Ability to learn quickly and adapt to shifting priorities.

  • Comfortable working with and learning new data systems.

  • Flexible and able to manage multiple tasks simultaneously.

  • Highly motivated with excellent and strong organizational, interpersonal, team building, critical thinking, and conflict resolution skills.


 


Education/Experience



  • Two years' experience in a general administrative staff or specialized staff capacity analyzing and making recommendations for the solution of problems of organization, systems and procedure, program, facility planning, budget or personnel -OR- One year's experience as an Administrative Assistant I or Staff Assistant I.


 


Certificates/Licenses/Clearances



  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

  • Successful clearing through the Live Scan process with the County of Los Angeles.


 


Other Skills, Knowledge, and Abilities



  • Excellent written and verbal communication skills, including the ability to collaborate

  • Knowledge of LA County programs that are designed to serve the most vulnerable clients.
