What You'll Do:

• Oversee and manage the social services department, ensuring residents' psychosocial needs are met.


• Develop and implement comprehensive discharge planning processes, coordinating with residents, families, and interdisciplinary team members.


• Connect residents and their families with appropriate community resources and support services.


• Provide counseling and support to residents and their families, addressing emotional, social, and psychological needs.


• Maintain accurate and timely documentation of social services assessments, interventions, and outcomes.

• Degree in a health-related field (Social Work, Psychology, Nursing, or related discipline).


• Strong knowledge of the discharge planning process and community resources.


• Previous experience in a healthcare or long-term care setting preferred.
