Job Title: RN - Home Health



Duties & Responsibilities: Provides individualized nursing care/assessments at the home/residence of the client diagnosed with acute, chronic and complex conditions.


A. Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.


B. Provides individualized care to clients with complex care needs including pediatrics, geriatrics, and mental health patients.


C. Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care.


D. Demonstrates the ability to care for clients with a variety of diagnosis' including but not limited to orthopedic, surgical, medical, cardiac, wound/ostomy, and home infusion needs.


E. Provides health coaching/education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent visit usage.


F. Delivers nursing care for the clients in their homes, utilizing appropriate available resources.


G. Involves the client/caregiver in goal development and plan of care development.


H. Involves the client/caregiver in developing and carrying out the plan of care.


I. Educates client/caregiver on disease process, treatment plan, and methods to increase independence for discharge planning. Evaluates and documents client's physical and emotional reactions to treatments, care, and medications in the electronic medical record (EMR).


J. Conferences with other disciplines appropriately to provide an interdisciplinary approach to meet all identified client care needs.


K. Assesses existing and potential problem areas, collects data, and reports significant information to the Clinical Manager or Clinical Leader.


L. Functions purposefully in and adjusts to emergency situations as they arise.


M. Instructs and supervises the home health aide, LPN, or other services provided to the client in their home requiring an RN supervision.


N. Submits and maintains accurate, current, and complete documentation according to policy on a timely basis.


O. Performs other responsibilities as requested by leadership. .


P. Maintains required department confidentiality and abides by HIPPA regulations.


Q. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.



Minimum Requirements:

• RN license (Iowa or Compact)


• Completion of ADN required, BSN preferred


• 2 years acute care patient setting


• Home health, community health, or hospice experience preferred


• Previous experience with mental health patients preferred, but not required


• Must be willing to drive in winter weather


• BLS


• Must provide proof of Mandatory reporting for child and dependent adult abuse training prior to employment or within 6 months of hire and maintain current status per Iowa guidelines


• ACLS (within 6 months of hire and maintain status)


• PALS (within 6 months of hire and maintain status)


• NRP (within 6 months of hire and maintain status)


• Completes MANDT training within 6 months of hire and maintains current status


• TNCC within 6 months of hire and maintain status


• Proof of and maintain valid drivers license and adequate insurance coverage on file
