raising the band to minimum $49,720; target max $62,150.

Purpose



The Case Manager provides assistance to Afghan SIVs, refugees, humanitarian parolees and other qualified immigrants to ensure newcomers are connected to resources and services. The Case Manager provides intake and assessment to program participants, developing a service plan and connecting clients to appropriate agencies and resources. The Case Manager works to support individual client needs and preferences offering a range of educational, financial, medical, social, vocational, and other services and resources based on needs and client eligibility. The Case Manager will operate out of the CWS Washington DC office with travel to local government and community service agencies. Case Managers work directly with incoming families and will be required to complete a background check, confidentiality agreement, and training modules, treating all individuals with understanding and respect.



Responsibilities




Primary Tasks:



  • Ensure the best possible service experience for each client, providing appropriate case management to connect newcomers with benefits and resources.

  • Conduct intake interviews and client assessments for Afghan clients to determine program eligibility; create service plans to meet the needs of individual clients and families.

  • Provide appropriate and timely referrals to ensure clients receive individualized and group orientations; Assist clients to access social services, health and mental health, and immigration legal services by assessing and evaluating each client's distinct needs.

  • Support clients with direct assistance funds distribution and tracking; Provide donations and other forms of assistance, as needed.

  • Ensure that clients are provided the basic tools necessary to meet their established short-term goals and objectives.

  • Assess and monitor client progress at regular intervals to ensure progress on goals is made, designated resources are maximized and modify initial resettlement plans and other case file documents as required.

  • Ensure compliance with case file documentation and reporting requirements, including timely data entry in hard-copy files and databases.

  • Travel regularly throughout the service delivery area (home visits, agency visits, service provider visits etc.).

  • Participate in all program meetings, staff development activities and fully engages as a member of the team.

  • Other related duties as assigned.




Qualifications




Education and Experience:



  • Bachelor's degree in social work, anthropology, health or related field.

  • 1 year experience working with refugees or immigrants, and previous case management experience.


Other Skills:

  • Strongly requires that candidates are bilingual in English/Dari/Farsi.

  • The successful candidate must be computer literate, with proficiency in MS Office Suite (Word and Excel) and internet applications.

  • Exceptional organizational and time management skills are required.

  • Excellent communication skills are essential.

  • Ability to work in a multi-cultural environment required.

Special Requirements





  • The successful candidate must pass a background check.

  • Must be willing to work occasional evenings and weekends.

  • This position is primarily in the office and community.

  • During busy times other duties may be assigned.
